squid3 TCP/denied - Linux

大家好

我是要用 squid3+pam 來設定 proxy，照這個網頁設定好之後

http://zeldor.biz/2013/03/squid3-pam_auth/

是可以通過驗證

但是存取速度很慢

甚至好幾次 timeout

查看 log 檔之後發現好幾次都是 TCP/miss 跟TCP/denied

請問是不是還有哪些設定我沒做的？
